Through the registration form or by phone — entirely without obligation. You tell us briefly about your situation.
A behavioural specialist and a support worker are present at the intake meeting; after that you meet your personal support worker.
After a period of observation, in which we really get to know you, we draw up the final care plan together.
Your regular support worker gets to work with you. We review the care regularly with everyone involved.
Several municipalities in our working area thank family carers with a gift voucher or flowers. The application deadlines are coming up soon — in Heumen and Mook en Middelaar as early as 1 October.
