Who processes your data?
Divers Zorg B.V. is responsible for processing your personal data. Our address is St. Agnetenweg 45, 6545 AT Nijmegen, and we are registered with the Dutch Chamber of Commerce under number 91072409. Do you have a question about your data? Call 06 4849 6899 or send an e-mail to info@divers-zorg.nl.
When do we receive your data?
In three situations. When you fill in the application form, when you send us a message through the contact form, and when you leave a call-back request. If you apply on behalf of someone else — as a family member, informal carer or referrer — we also process that person's data. If you then become a client with us, we record further data that we need in order to provide good care.
Which data is this about?
Through the forms: your name, phone number and, if you fill it in, your e-mail address, date of birth and town. The description of the care question often contains information about someone's health or situation. That is special category personal data, which has extra protection; we therefore handle it with extra care and do not share it unless it is necessary.
Why are we allowed to do this?
For the forms on this website we rely on your consent: you tick the box yourself, and you can always withdraw that consent. If you are a client, we process data because it is necessary to carry out the care agreement with you, and because the law requires us to. We never use your data for advertising and we do not sell it.
Who can see your data?
Within Divers Zorg only the staff who need your data for their work. We also work with a few parties that help us with this: the company that keeps this website running, and the party that sends our e-mail — an application reaches us by e-mail. We make agreements with these parties about how they handle your data. Apart from that we share nothing with others, unless you give permission or the law requires us to.
The map on our contact page
The contact page has a map from Google. We deliberately do not load it automatically: first you see a button. If you click it, Google is shown your IP address and the page you are on. If you do not click, nothing happens. That way you decide for yourself.
What does this website store?
We do not use cookies to track you and we do not measure your visit with analytics software. If you switch on 'Increase contrast' or 'Larger text', your browser only remembers that choice, so the setting stays in place when you come back. That information stays on your own device and does not reach us.
How long do we keep your data?
No longer than necessary. If an application does not lead to care, we delete the data as soon as it is clear that we cannot do anything for you. If you do become a client, legal retention periods apply to the care file. Which period applies depends on the law your care falls under. Would you like to know how long we keep your data? Feel free to ask us; we will tell you exactly what applies in your situation.
How do we protect your data?
We take appropriate technical and organisational measures. The connection with this website is encrypted, only authorised staff have access to files, and our staff have a duty of confidentiality.
Your rights
You may view your data, have it corrected or have it deleted. You can also ask us to limit the processing, object to it, or have your data transferred to another party. If you gave consent, you can withdraw it at any time; what happened before that remains lawful. Please contact us for any of these requests — we respond within one month. If you are not satisfied with how we handle your data, you can file a complaint with the Autoriteit Persoonsgegevens (the Dutch data protection authority) via autoriteitpersoonsgegevens.nl.
