Do you work as a GP, in a neighbourhood team, at the municipality or at the zorgkantoor (the regional care office)? On this page you will find our referral route, our care services and the direct contact details for consultation.
Referring in three steps
We deliberately keep the route short. One phone call is enough to check whether a case fits with us.
A first consultation by phone
Call us for a short first consultation about your case. We discuss the care question, the client's situation and whether our services are a good fit — with no obligations yet.
Registration
Is it a good fit? Then you register the client with our online application form. Please state the care question and, if known, the assessment or the funding.
Intake and observation period
A behavioural specialist and a support worker carry out the intake. After that comes an observation period, and then the support plan is drawn up. As the referrer, you stay informed.
Types of care and funding
Divers Zorg offers eight types of care for adults with a mild intellectual disability, mental health problems (GGZ) or dementia, and for clients with a multicultural background.

Wmo
The Dutch Social Support Act (Wet maatschappelijke ondersteuning) — the decision (beschikking) is granted by the municipality.
Wlz
The Dutch Long-Term Care Act (Wet langdurige zorg) — based on an assessment by the CIZ, the national body that assesses long-term care needs. Personal contribution (eigen bijdrage) via the CAK, the body that collects it (hetcak.nl).
pgb
Persoonsgebonden budget — a personal budget you use to buy your own care. Payment and accounting run through the SVB, the bank that pays care providers from your pgb.
Which form of funding applies depends on the assessment and on the client's situation. In the first consultation we are glad to think along about the right route.

Availability
Availability differs per type of care. Call us for the current options — we are glad to think along, also when a case does not fit our services one to one.
Is a placement with us not suitable? Then we will say so clearly, so that you can move on quickly with your client.
