When several care workers and organisations are involved in your situation, the overview can disappear quickly. Who arranges what? Which agreements have been made? Case management solves that: one regular case manager keeps an overview of all your care and support, coordinates the organisations involved and makes sure that agreements are kept. That way you and the people close to you always have one familiar contact person.
Case management is valuable for adults in the Nijmegen region whose care is complex — for example with dementia, with mental health problems (GGZ), or with an intellectual disability combined with other questions for help. With care for dementia in particular, a regular case manager brings rest: he or she knows your story, notices changes in time and keeps in touch with your doctor, the municipality and other partner organisations, so that the care keeps fitting.
At Divers Zorg, case management starts, like all our care, with an intake by a behavioural specialist and a support worker. Your case manager gets to know you and your network during a period of observation and then draws up a plan together with you. The progress is evaluated in a multidisciplinary review, so that everyone involved follows the same course. Do we also provide other kinds of care for you, such as individual support or personal care? Then the case manager makes sure that everything fits together.
You pay for case management at Divers Zorg with a pgb (a personal budget you use to buy your own care), which you apply for under the Wmo (the Dutch Social Support Act, arranged by your municipality) or the Wlz (the Dutch Long-Term Care Act).
