Individual support

Personal support with structure, independence and social contacts.

Individual support — in Dutch called ambulante begeleiding — is personal help with the things in daily life that do not go by themselves for a while. Think of giving your day some structure, sorting out your paperwork and post, building up social contacts, or finding your grip on life again. A regular support worker visits you at home, or meets you somewhere you feel comfortable, and works with you on goals that matter to you.

This kind of support is there for adults in Nijmegen and the Rijk van Nijmegen region with, for example, a mild intellectual disability (in Dutch: LVG), mental health problems (GGZ) or early dementia. People with a multicultural background, including status holders (refugees with a residence permit) and asylum seekers, are welcome as well. Social work often thinks along with you briefly on practical questions; our individual support is aimed at the longer term — close to the way you live, with an eye for your culture and your background.

At Divers Zorg, the support starts with a careful intake by a behavioural specialist and a support worker. After that comes a period of observation in which we really get to know you, so that the support plan fits who you are. We work from the presence approach: first a bond of trust, then goals. We discuss your progress regularly in a multidisciplinary review, and where needed we adjust the plan together with you.

You arrange our support with a pgb (a personal budget you use to buy your own care). You apply for that budget under the Wmo (the Dutch Social Support Act, arranged by your municipality) or, if you need care for a long time, through the CIZ (the national body that assesses long-term care needs) under the Wlz (the Dutch Long-Term Care Act). We do not have a contract for care in kind (zorg in natura), so with us it always runs through a pgb. Not sure which scheme applies to you? How do I apply for individual support in Nijmegen?

You can register with us directly through the registration form or by phone on 06 4849 6899. If the care is paid from the Wmo, the application runs through your municipality; we are glad to help you with that step. After you register, we plan an intake with a behavioural specialist and a support worker.

What does individual support cost?

At Divers Zorg you always arrange the care with a pgb; we do not have a contract for care in kind. You apply for the budget itself under the Wmo through your municipality, or through the CIZ under the Wlz. Which route applies to you depends on your situation. We are happy to look at it with you without obligation, and we help with the application free of charge.

How often does the support worker come by?

We agree that together, and it depends on your goals and your indication (the decision that says which care you may receive). For one person one moment a week is enough, another needs several contact moments. During the multidisciplinary reviews we look at whether the frequency still fits.
